SD43 application and review
Apply to become a Program Provider Partner with SD43-Afterschool Guide and offer affordable programs supporting children and youth in collaboration with the SD43 Community Connections Team.

Complete the partnership application
Submit the SD43 Community Partnership application form.

Submit a Program Proposal
Use the Program Proposal form for each program you would like reviewed.

Complete the SD43 review
SD43 reviews applications, proposals, references, policies, and program guidelines and may schedule a call or meeting.
Other required information
- After proposal approval, submit the Provider Info form to confirm legal status, insurance status, and organization details.
- Submit a Certificate of Liability Insurance through the Liability Insurance form.
- Submit an Instructor Info form for each instructor, including Criminal Record Check status for SD43 programs.
- Review and sign the Letter of Understanding sent to the Provider.
- Make the SD43 Instructor Guide and safety requirements part of mandatory staff training.
- Once the requirements are complete, approved programs can be included in the vetted SD43-Afterschool network and shared with schools.
DWUL Community Platform
- Create a Provider account on DWUL.io and complete your public provider profile.
- When a school request is confirmed, post the applicable programs on DWUL for registration.
- Use the Provider account to manage listings, school communication, registration, student lists, payment reporting, subsidies, and administrative support.
